Discharge and Referral - the information gap

Primary Healthcare Specialist Group Annual Conference

This years annual conference will consider just how far the goals of creating integrated health and social care systems have been achieved, contrasting the differing approaches of England, Wales, Scotland and N. Ireland in order to determine what can be learnt from the experiences gained so far, as well as an assessment of ‘the gaps’ in information (and knowledge representation) that remain.
Whilst true ‘interoperability’ between systems remains elusive, it is hoped that practical examples of integration between Primary, Community and Acute care systems will be demonstrated, highlighting the technical and ethical barriers that have been overcome in the quest to provide more comprehensive continuity of care for patients. The role of the patient in assessing and potentially augmenting information held in their care record will also be explored.
